La recomandarea Direcției de Sănătate Publică Constanța, punem la dispoziția tuturor celor interesați următorul chestionar de evaluare nutriție:

Chestionarul de evaluare nutriție poate fi completat de orice persoană, instituție sau organizație.

Durata de completare a chestionarului: aproximativ 3 minute.

Rezultatele vor fi incluse în Campania IEC Nutriție din luna octombrie 2021.
